Managing your health can feel like a full-time job, with appointments, medications, insurance, and paperwork all at once. Our Care Management program pairs you with a dedicated care manager who gets to know you, understands what matters to you, and helps you bring all the pieces together.

What your care manager does with you:

• Builds a care plan around your goals, not just your diagnoses
• Coordinates with your doctors, specialists, behavioral health providers, and pharmacy so everyone is on the same page
• Helps you schedule appointments, follow up after hospital or ER visits, and keep track of medications
• Connects you to services like home care, transportation, and community supports
• Checks in regularly, by phone, in person, or wherever works best for you

Who it’s for:
Care Management is for adults and families living with chronic health conditions, behavioral health needs, or both, who could use extra support navigating care.
