Care Management

Care Management, also known as care coordination, is a program designed to support members in their management of chronic health conditions so that they have positive health outcomes and live a better quality of life. Under the care management program, members are assigned a dedicated care manager who coordinates services and addresses social determinants of health including healthcare, employment, housing, and other community resources.

A care manager can help:

  • Make appointments with your doctors and/or specialists to ensure you get the best care possible.
  • Help you understand chronic health conditions
  • Assist you with applications, recertifications, legal resources.
  • Assist you with housing applications
  • Coordinate your transportation and more
